Are there king size beds in the balcony state rooms?

Read the description of the rooms. Some of the spa rooms specifically say they have king size beds. Most rooms say they have two twins that can be moved together for a queen size bed.

is the behind the scenes tour free

It is free if you are platinum or higher but you will have to register for it at the cruise next desk and make sure to do it early as it will fill up quick. The free tour is also different than the paid one you book from the excursion desk, when we went on the free tour we viewed the backstage in the theater, laundry area, and food prep areas.

No. Laundry service is available and charged either per item or by the bag, similar to when you stay at a hotel. It is typically a 24-hour turn around.

Take NCL pool towels on excursions?

Once pool towels are checked out, can they be taken on excursions?

Typically yes, since most travelers don't bring their own beach towels on a cruise with them. You can sign out towels. They usually have someone documenting your cabin number when you sign out/turn in towels. Just make sure that if you take towels with you on an excursion that you bring them back to the ship or you may be charged for towels. Same for the pool. There is a pool exchange by the pool. You turn in a wet one and you get a dry one back.

I am from the US, do I need European converters for our electric items?

There are US (110) and European (220) outlets in all staterooms. There are not many, nor are they always conveniently located. An extension cord or power strip can come in handy.

How noisy is 14th floor?

I'm looking at a room directly underneath the Sun Deck on 15th floor. I'm wondering if you can hear deck chairs scraping as people drag them across the deck?

Does the Breakaway have water fountains? If I wanted to bring my refillable water bottle?

No water fountains but there are waster dispensers in the buffet you can use. Staff will ask you to use a cup to fill with water first instead of putting your bottle directly on the dispenser.

Are there USB charging ports in the cabins?

Not when we last cruised on Breakaway but they may have upgraded since then. And even if they did add USB ports, there will likely not be enough. Your best bet is to get a travel adapter with multiple USB ports that plugs into a standard 110 outlet, Amazon has a bunch to choose from.
