Are there recommendations for activities at each port other than the excursions?

Not sure about this specific cruise, but others I have been on typically have stands throughout where you can purchase excursions separately. However, I do recommend purchasing excursions through the cruise line if you can. They are usually more trustworthy, and they will hold the ship for you should they learn that any excursion is running late returning from an outing.

I will be in handicap accessible balcony room. Where will I put my cpap, plug it in and get distilled water? Can I bring extension cord?

We have sailed on four different NCL ships with two CPAP machines. Have always requested distilled water in advance, and one time out of the four it was actually waiting for us. The other times we had to ask our steward who quickly brought it. Usually a gallon, but on our most recent cruise in Europe it was five liters! Way more than you need! Depending on cabin, there may be outlets by the bed but often you'll need to run a cord to the desk. We've always brought a 12 ft. extension cord with a multi-outlet tap on it and never had an issue. They MAY confiscate a power strip with surge suppression so play it safe and look for a cruise-safe one online. The bedside tables have barely enough room for a CPAP so don't count on too much room for anything else.

I am cruising from Seattle to Alaska. How many hours before this cruise, do I need a covid test?

According to their guidelines: "At time of check-in, all guests will be required to provide proof of a negative COVID-19 antigen or PCR test result administered by a verified third party or via medically supervised home test within two days prior to embarkation date for cruises originating in a U.S. port and within three days prior for voyages departing from a non-U.S. port."

How many outlets are there in a balcony stateroom on the Norwegian sun? Am I allowed to bring a non surge protector power strip?

We cruised the Sun in 2016 so my info may be out of date. As an older ship, there was one outlet of each type (U.S. and European) at the counter across from the bed. We had adapters for each plug so were able to provision two, and used an extension cord with multiple outlets to power two CPAP machines next to the bed. More recently I've brought a non-surge suppression power tap onto the Spirt and Epic and never had an issue with it.
